Medical Editing Solutions provides digital transcription options using Bytescribe DocShuttle products and back-end speech recognition editing/authentication within the award winning suite of dictation products developed by Provox Technologies. Medical Editing Solutions maintains SQL products and an ftp site for the secure transfer of files directly to your site. We run Provox on our servers because we believe in the productivity of speech recognition. These same servers have capacity for digital dictation using .wav, .dss, .mp3, .avi and any other sound files.
Accordingly, MES can access, via secure ftp, VPN or SQL replication connection, off-site servers/software at healthcare institutions across the United States where appropriate privileges and bandwidth are established. This access provides connection to both .wav, .dss and .vox files stored locally at your office/hospital location or on our internet site. This provides real time download, transcription and editing of provider's digital dictation files. Digital dictation files can be created using Olympus brand digital dictation products, telephony or created in Provox dictation suite. The definition/workflow for these dictation files are listed below:
.wav, .dss Files: DocShuttle and Provox Technologies offer standard .wav files in all of their speech recognition/dictation products. These files are attached to specific visit/accession numbers, visit numbers, dates of service, for example, within the Provox database since their products are typically interfaced with your institution's RIS / PACS or HIS via HL-7 messaging. Dictation .wav files are typically created based on a user's choice not to engage speech recognition on a per report basis. These files are stored on your local servers at the database level. This allows MES to access these files via a remote user license for rapid transmission and report turnaround. When using the .wav feature in Provox, the user simply selects a name off the work list and begins dictating as they normally do. This “per report choice” will bypass speech recognition in the event the provider is unable or simply not inclined to use SR for a given report(s).
.vox Files: Provox Technologies also offers .vox files in all of their dictation products. These files are also attached to specific accession numbers, visit numbers, dates of service, etc via HL-7 or DICOM interfaces with the RIS / PACS or HIS. These dictation .vox files are typically created based on a user's choice to engage speech recognition at the point of dictation. The choice can be made globally or on a per report basis. These files are stored at the database level on your local servers. This allows MES to access these files via a remote user license for rapid transmission and report turnaround. When using the .vox feature in Provox, the user simply selects a name off the work list and begins dictating. Speech is instantly turned to text on screen at accuracy up to 98%. Upon completed dictation, the user may lock the report in real time, or “defer” the report for MES editors. Medical Editing Solutions monitors all deferred reports in real time, downloads a copy of these files via remote access, corrects the deferred report, and sends it back to the provider for final review and signature. All corrections made at the MES office to these files improves the speech recognition accuracy of the provider who dictated the report at the healthcare institution. Thus, since all reports are corrected in the award-winning Provox speech recognition workflow, accuracy of most providers dramatically improves WITHOUT their direct participation. In this manner, speech recognition accuracy improves often to 95% or better via MES editing protocols within 100-200 reports. This enhances the statistical probability that a large percentage of daily reports will fall into a category of “dictate and done,” whereby the report need not be sent for edits. The result is both cost and time savings. It is also a comfort to most providers that this workflow will allow them to migrate to the speech recognition with little or no growing pains.
The above work flow examples can also be enhanced by other features in Provox named SmartMacros. These macros are user defined and can be distributed at both a public and private level via the Provox application. In this manner each provider can build and maintain a database of normal reports or normal report sections. These macros are often automatically triggered by the Provox application via an interface with the PACS, or via a simple Provox command. MES specializes in the creation of these macros for our clients to improve efficiency and reduce costs. These SmartMacros can be combined or associated with traditional typing (.wav files) or speech recognition (.vox files) editing inside the Provox product suite to further reduce report turnaround and costs.
